Integrated Diabetes Services Terms of Consultation
Integrated Diabetes Services and its staff and owners (IDS) provides diabetes education and management consultations based on up-to-date standards of diabetes care, including those contained in the CORE Curriculum for Diabetes Education and the American Diabetes Association’s Clinical Practice Recommendations. Reasonable steps have been taken to make sure that all IDS representatives are properly trained and equipped to provide appropriate professional guidance. IDS is not a medical practice and there are no physicians or other licensed medical personnel on staff at IDS. IDS does not prescribe or dispense controlled substances or any other medications. Our role is to supplement (not replace) the services normally provided by your physician/medical providers.
As our client, you are advised to use your best judgment and consult the physician/medical providers who prescribe your medications prior to implementing any of our recommendations. We also ask that you keep IDS informed of therapy changes made by your physician/medical providers.
Diabetes is a complex condition that may result in acute and chronic complications. Recognizing that blood glucose regulation is a very inexact science, IDS cannot be held responsible for any health problems that may arise as a result of our recommendations. You hereby agree to release, indemnify and hold harmless IDS from any and all damages, costs or expenses (including reasonable attorney’s fees) which you may incur in connection with our recommendations (absent gross negligence). In addition, the undersigned will not hold IDS accountable for the performance of devices used in the course of diabetes management, including but not limited to blood glucose meters, insulin pumps and related software.
IDS will not share information about you or your medical background with anyone, including your family, employers, health insurers, pharmaceutical companies, medical product manufacturers and market research companies, without your prior authorization.
IDS makes every effort to respect your time and keep waiting to a minimum. Please respect our time and the needs of other patients by letting us know if you are unable to keep a scheduled appointment. To help keep fees reasonable for everyone, we will bill at our usual rate for all missed appointments (when we are not notified at least one hour prior to your scheduled appointment time).
Payment for all services is due when services are rendered. New (first-time) appointments must be secured by check or credit card at the time of scheduling. Retainer services go into effect at the time payment is received. Refunds are provided at the discretion of IDS.
IDS reserves the right to refuse service to anyone based on ethical and sound business practices.
