We’re excited to share that scheduling with Integrated Diabetes Services is now easier than ever!
Our new online system allows both new and returning patients to book appointments quickly and conveniently.
Step-by-Step Guide for New Patients:
- Visit the New Patients Page:
https://integrateddiabetes.com/new-patients/ - Choose Your Clinician:
You can use the clinician matching tool to find the clinician best suited to your needs. - Select a Date and Time:
Pick a slot that works best for your schedule. - Enter Patient Information:
If the patient is a child, please include the child’s name and the caregiver’s phone number and email. - Provide Payment Information:
You can pay using credit cards, PayPal, or HSA/FSA cards. - Confirm Your Appointment:
Once confirmed, you’re all set! - Complete the Diabetes Assessment Form and Terms:
The final step is filling out the diabetes assessment form, reviewing the terms of consultation, and consenting to data sharing with our clinic. Links for these documents are included in the new patient email you will receive. Complete 3 days before your appointment.
For Returning Patients:
If you haven’t met with your clinician in a while, this is a great opportunity to reconnect and get back on track with your diabetes management. We can help with glucose trends, technology, or day-to-day management strategies.
Follow-Up Retainers:
If you’re interested in setting up a follow-up retainer, please contact Amy at amy.sczepanski@integrateddiabetes.com to request a scheduling link for retainer packages.
If you prefer, you can also call our office at 610-642-6055 (option 1 for Amy) for help with scheduling or any questions. We’re always working to make scheduling as smooth as possible. This new system gives you more flexibility and control when booking your care.
Please reach out if you have any questions — we look forward to supporting you!
Best,
Amy Sczepanski
