How to Share Your Diabetes Device Data with Integrated Diabetes Services

To help us provide the best care, please share your diabetes technology data with Integrated Diabetes Services.

Below are step-by-step instructions for the main platforms. 

Please follow the directions for each system you use.

Carelink (MiniMed)

  1. Log into your CareLink™ Personal Account.
  2. Go to your Account, Profile, or Sharing settings.
  3. Select the option to Link Care Partners or Connect with Healthcare Provider.
  4. Enter the Carelink Clinic ID: 12047

Open Source AID: Loop & Trio

  1. Please send an email to info@integrateddiabetes.com with the patient’s first and last name, Nightscout URL and API or Token.

Dexcom Clarity

On the computer:

  1. Go to https://clarity.dexcom.com
  2. Log in with your Dexcom account credentials.
  3. Click on the ‘Settings’ or ‘Sharing’ option.
  4. Select ‘Share Data with Clinic’.
  5. Enter the sharing code: integrated
  6. Confirm sharing and save.


On the app:

  1. Login into Dexcom Clarity
  2. At the bottom right, click on “Profile”
  3. Click on “Manage Data Sharing”
  4. Enter the clinic code:  integrated
  5. If a list is presented, Choose “Integrated Diabetes Services”

Glooko

  1. Go to https://my.glooko.com and log in to your account.
  2. Navigate to ‘Account Settings’ → ‘Clinic Sharing’.
  3. Enter the clinic code: integrated

LibreView

  1. Open the App: Launch the FreeStyle LibreLink or Libre 3 app.
  2. Navigate: Tap the Menu (three lines) > Connected Apps.
  3. Connect: Tap LibreView, then Connect or Manage.
  4. Add Practice: Select “Add Practice” or “Connect to a Practice” and enter the ID: 04799947
  5. Confirm: Follow the prompts to complete the connection

Tandem Source

  1. Log in to your Tandem Source account at:  https://source.tandemdiabetes.com
  2. Click on your initials in the top right
  3. Click on “Account Settings.”
  4. Click on “Share Reports.”
  5. Click on “Add Clinic”
  6. Choose the state of Pennsylvania and type Integrated Diabetes Services. (For Canadian clients, use Integrated Diabetes.)
  7. Select our clinic and confirm.

Tidepool

  1. Go to https://tidepool.org and log in to your account.
  2. Click on ‘Share’ or navigate to the sharing section.
  3. Enter the clinic share code: Y9TM-4LBX-R8AL
  4. Confirm and save.
